About Karel Winner

The prescriber behind this practice: doctorally prepared, measurement-based, and plain-spoken about what medication can and cannot do for drinking.

Karel Winner, DNP, PMHNP-BC, psychiatric prescriber for alcohol use disorder medication management

Karel Winner is a doctorally prepared psychiatric nurse practitioner. His Doctor of Nursing Practice is from Vanderbilt University, his psychiatric training from the University of Tennessee, Knoxville, and he holds a graduate degree in nursing education. His clinical background spans two decades in medicine: podiatric medicine and surgery, vascular ultrasound, and psychiatry.

He founded Solutions Psychiatry to practice psychiatric medication management the way he believes it should be done: a real evaluation before any prescription, a safety screen where safety is the question, honest measurement at every step, and follow-through when the first plan is not the one that works. The approach on this site is the approach in the room: count honestly, screen for safety, choose the medicine by the goal, measure the result, and always know the next step before you need it.

Alcohol use disorder is where that approach matters most, because it is the most undertreated condition in psychiatry. Almost nobody who has it is offered the medicines that treat it. This practice offers them, carefully, with the count and the safety screen doing the talking. Two further commitments run underneath the clinical work: deprescribing where a medicine has become part of the problem, and plain language everywhere, because a patient who understands the plan is a patient who can run it.

Dr. Karel Winner, DNP, PMHNP-BC, NE-PMC, CAMS-II, CTP, IFS-L3, BSP-P5, CLC, PATP, ADHD-CCSP, ASDCS, CYMHS, RDMS, RVT, RCPod. NPI 1477238004. Quit Drinking Meds is a service of Solutions Psychiatry.

Two decades, three fields

The route to this practice was not a straight line, and the practice is better for it. Karel Winner trained first in podiatric medicine, earning honours in podiatric medicine at the University of London in 2013 and practicing podiatric medicine and surgery. He then spent seven years in vascular ultrasound at Santa Clara Valley Medical Center, from 2016 to 2023, as a sonographer and clinical instructor, which is where the measurement habit was formed: image first, measure honestly, and let the numbers argue. Nursing followed at Alverno College, a psychiatric nurse practitioner post-master's at the University of Tennessee, Knoxville in 2023, psychiatric work at Aurora Psychiatric Hospital, Ascension, and Hope Mental Health, and then the Doctor of Nursing Practice at Vanderbilt University, completed in 2026, alongside a graduate certificate in nursing education.

He founded Solutions Psychiatry in 2023 to practice prescribing the way those twenty years argued it should be practiced. Evaluation before prescription. A safety screen wherever safety is the actual question. A count kept plainly enough that a patient can check the prescriber's work. And a plan for what happens when the first plan fails, written down before it is needed, because in alcohol treatment the second plan is the one most people end up living on.

Why alcohol care, specifically

Of everything in psychiatry, this is the condition where the gap between what exists and what gets offered is widest. Effective, non-controlled, inexpensive medicines exist for alcohol use disorder, guidelines support them, and almost nobody is offered them; the national figures are on this site's home page, with their source named. A prescriber who measures carefully and prescribes without drama can close a real part of that gap one patient at a time, and that is the work this lane of the practice exists to do.

The practice

Solutions Psychiatry is the practice behind this site. Patients can also reach it through psych.us for therapy and the full range of psychiatric care, psychmedcare.com for general psychiatric medication management, and through the OCD medication management lane for that condition. Quit Drinking Meds is one practice with a specific door: alcohol use disorder medication management, for people in 34 states and DC.
