Costs and insurance
Costs lead with insurance on this page, because that is how most patients here pay for alcohol use disorder medication management. Your coverage and your exact cost are verified before your Comprehensive Initial Appointment, so you know your copay before you are seen.
This practice is in network with Aetna, Cigna and Evernorth, UnitedHealthcare and Optum, Blue Cross Blue Shield, Oscar, Oxford, Carelon, and Providence, and with additional plans beyond that list. With insurance, copays typically run $0 to $40 per visit. You will know your exact figure before your first appointment, not after it.
There is a reason this page is organized this way. Alcohol treatment is usually priced in the dark: programs quote a month at a time, directories list fees nobody has verified, and patients learn what they owe from a bill. Care here is two appointments with two prices, and the insurance path is checked against your actual plan before anyone is seen. If the verified answer turns out to be worse than you can carry, you learn that before the appointment, when the information can still change your decision, not after it.
Verified means four specific things, checked against your plan itself. First, that the practice is in network for your exact plan, not just for the insurer's name on your card, because the same insurer sells plans this practice is in network for and plans it is not. Second, your copay or coinsurance for a medication management visit, stated as a number, not a range. Third, whether your deductible has been met, because an unmet deductible can make an in-network visit cost more than the self-pay rate, and you deserve to know that before you choose. Fourth, whether your plan wants any authorization before the first visit, so the paperwork is done before the appointment rather than argued about after it.
Verification is also repeated when it matters. Plans change at renewal, jobs change mid-year, and a copay verified in January can be wrong by June. When you report a new card or a new plan, the check runs again before your next visit, and the new figure replaces the old one in writing.

Credentialing expands constantly, so this list only grows. Your specific plan is verified before your Comprehensive Initial Appointment. Not sure? Email [email protected] with your plan name and we will check.
If your insurance isn't listed above, you may still be covered. Many plans reimburse out-of-network care, and this checks yours in about a minute, right here, secure and HIPAA compliant.

Medicare is accepted in Arizona, California, Colorado, Texas, and Utah. Medicaid is accepted in Oregon and Wyoming. If you are on Medicare or Medicaid outside those states, the estimator above will route you to the information line rather than guess at your cost.
Government coverage follows its own rules, and those rules are state-shaped. Medicare acceptance here covers five states because licensure and enrollment have to line up state by state; a Medicare card from a sixth state does not become usable because the visit is by video. Medicaid is narrower still, Oregon and Wyoming, because each state's Medicaid program enrolls providers separately. If your coverage is Medicare or Medicaid outside those states, say so when you write to [email protected]. The answer will be plain about what is possible now, and your state may be one of the ones in progress.
The visit is not the only cost in medication treatment, and pretending otherwise would be its own kind of surprise. The medicines themselves are filled at your pharmacy under your plan's pharmacy benefit, which is a separate line from the visit copay. The medicines used for drinking are long-established and available as generics, stocked by every ordinary pharmacy, and for most patients they are among the least expensive prescriptions they fill. Your exact pharmacy cost depends on your plan's formulary, and if a chosen medicine lands on an expensive tier, that is raised at the appointment, where a different medicine in the same class can usually be chosen instead. Nobody here learns what their medicine costs from the pharmacy counter if it can be helped.
Plans sometimes decline a medicine, a visit type, or an authorization, and a denial is a paperwork event, not the end of the plan. The practice handles the appeal and the documentation, because the clinical record that justifies the care is the practice's to write. While an appeal runs, the alternatives are stated plainly: a different medicine in the same class that the plan does cover, the out-of-network benefits check above, or the self-pay rates on this page, which do not change because an insurer is being difficult. What never happens is a denied service quietly billed to you at a rate you never saw.
The self-pay structure is deliberately boring. The Comprehensive Initial Appointment is a full hour and costs $350. Follow-up Appointments are 30 minutes and cost $250. There are no facility fees, no program fees, and no monthly memberships on top of them. The estimator above runs the same numbers you have just read. If your verified insurance cost ever comes back higher than the self-pay rate for the same visit, you will be told, and the choice of which way to pay is yours.
You book your Comprehensive Initial Appointment and send your insurance details through the booking flow.
Your plan, your network status, and your copay are checked against your actual coverage, not a guess from a directory listing.
You receive your exact cost before the appointment happens. If anything about the coverage changes the plan, you hear it before anyone is billed, not after.
