Why willpower was not the problem

If you are in crisis, call 911, call or text 988, or text HOME to 741741. This practice is not an emergency service.

Almost everyone who books a first appointment here has already run the willpower experiment, usually several times, with real effort and real early success. Then it failed. The conclusion they draw is about themselves. The conclusion the evidence supports is about the experiment: effort was being applied to a system that alcohol had already rewired, and rewired systems do not respond to effort the way intact ones do.

What alcohol does to the machinery

Alcohol pays out in the brain's reward system, and repeated payout teaches the system to expect it: cues get loaded, the same hour, the same route home, the same feeling at the end of a workday. At the same time, the stress system recalibrates. The brain starts producing less of its own calm and leans on alcohol to supply it, which is why the sober hours begin to feel like a held breath. Researchers describe the condition as a cycle: drinking, then a withdrawal state that feels worse than baseline, then preoccupation with the next drink. Each pass through the cycle strengthens it.

Why the attempts end where they end

This is also why effort-only attempts so often end around the same point, a few weeks in. Early motivation is a real force, and it carries the first weeks. What it cannot do is lower baseline craving, un-teach the cues, or rebalance the chemistry by wanting to. When the attempt fails at the same place every time, the failure has a mechanism, and mechanisms can be treated.

What medication does differently

The medicines for this condition aim at the machinery, not at the person's resolve: one class lowers craving, one blocks the reward payout so a drink teaches less, one steadies the system while it rebalances. None of them supplies willpower. They lower the amount of willpower the day demands, which is a different and much more achievable thing. Combined with an honest count, so the effect is measured rather than hoped for, that is the whole model.

The sentence worth keeping

If your attempts have failed, the accurate reading is not that you did not want it enough. It is that you were treating a brain condition with the one tool the condition specifically disables. Wanting it is the entry requirement, and you clearly meet it. The rest is mechanism, measurement, and time.
